Cancellation and Rescheduling Policy
If an appointment is cancelled or rescheduled within 48 hours of the scheduled appointment time, or in the event of a non-attendance (no-show), 100% of the appointment fee will be charged to the card held securely on file. Please note that cancellation and non-attendance fees are not covered by private health insurance providers (including Bupa, AXA, Aviva, WPA and Allianz) and will be charged directly to the client. If you are unable to attend due to illness or adverse weather, you may request to convert your appointment to a video consultation where clinically appropriate.
Insurance Information
Clients intending to use private health insurance must provide their insurer name, membership number, authorisation code, full name, date of birth and home address prior to booking. This information is required to verify cover and submit claims. If insurance details are not provided before treatment, appointments may be charged at the standard self-funded rate and may not be transferable to your insurer retrospectively. Any policy excess, shortfall or treatment not covered by your insurer remains your responsibility and must be paid within 48 hours of the invoice date. Failure to do so will result in the outstanding balance being charged to the card held securely on file.
Privacy Policy
All client information is treated with the highest level of confidentiality and is used solely for the purpose of providing physiotherapy and related healthcare services. Honeyrock Physiotherapy is registered with the Information Commissioner’s Office (ICO) and complies with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. We are committed to ensuring that your personal information is handled securely, responsibly and in accordance with legal requirements. Client information will only be shared with third parties where necessary for your care (for example, your GP or insurer) or where required by law, and only with appropriate consent where applicable.
Client Responsibilities & Agreement
By booking an appointment, you agree to:
- Provide accurate and complete health and medical information.
- Complete the client intake form, including valid payment card details, prior to your first appointment. Failure to complete the intake form at least 48 hours before your appointment will result in your appointment being cancelled and offered to another client.
- Inform your physiotherapist of any changes to your health status, symptoms or medication.
- Provide complete private health insurance details, including your insurer name, membership number, authorisation code, date of birth and home address, prior to treatment where applicable.
- Respect the cancellation and rescheduling policy.
- Pay all agreed fees, including insurance excesses, shortfalls and any fees not covered by your insurer, within 48 hours of the invoice date. Failure to do so will result in the outstanding balance being charged to the card held securely on file. By providing your payment card details, you authorise Honeyrock Physiotherapy to securely store your card and charge it only in accordance with these Terms and Conditions, including late cancellations, non-attendance, unpaid insurance balances and outstanding invoices.
